Make sure the option Comments Finding is set accordingly to your way of describing code.
If you have selected the Force token usage for code symbols check box on the [General Settings] > Documentation > Comment Finding > Token page make sure Tokens are the first thing in a comment. Notice that it is not possible to build walls around your comments if you are using Tokens.
If you have selected the Force token usage for code symbols check box and you want the first comment in your file to be associated with the file, you need to insert Tokens and the name of the file in front of the first comment.
