Doc-O-Matic creates a topic for each symbol by default. Whether there is documentation available or not is indicated by a green (for yes) and a red (for no) icon next to the respective symbol in the Content Manager. Doc-O-Matic assumes that there is a documentation available if a comment block can be associated with a specific symbol. However, the comment block may only consist of comment characters and ignored sections. To prevent Doc-O-Matic from displaying "empty" comment blocks as documented symbols you can use the Ignore Empty Topics feature. Using this feature you can treat topics that have no "real" documentation as undocumented (red icon).
